정맥혈전색전증에서 NOAC

Abstract

The results of trials using novel or non-vitamin K-dependent new oral anticoagulants (NOACs) in the treatment of venous thromboembolism (VTE) reveal that these agents are non-inferior (in terms of efficacy) and possibly safer (particularly in terms of major bleeding) than the standard heparin/vitamin K antagonist (VKA) regimen. High TTR values were achieved under VKA treatment in all trials; however, it should be noted that the study populations comprised relatively young patients, very few of whom had cancer. At present, NOACs can be viewed as an alternative to standard treatment. Currently, experience with NOACs is limited, but continues to be accumulated. Practical recommendations for the use of NOACs in different clinical scenarios and the management of their bleeding complications are needed. The results of the trials using NOACs in the extended treatment of VTE are in line with those of the studies that tested these agents for acute-phase treatment and standard duration of anticoagulation after pulmonary embolism (PE) or VTE. They indicate that NOACs are both, effective (in terms of prevention of symptomatic or fatal recurrence of VTE) and safe (particularly in terms of major bleeding), probably safer than standard VKA regimens.

서론

폐동맥색전증 및 심부정맥혈전증은 사망률이 높은 질환으로 빈도가 최근 들어 빠르게 증가하고 있다. 심부정맥혈전증과 폐동맥색전증은 동일한 범주의 질환으로 생각되고 있으며, 폐동맥색전증의 경우 초기에 적절한 치료가 필요하다. 급성기 치료 이후에 합병증을 줄이고 재발을 방지하기 위해 혈전증의 위험인자를 확인하여 적절히 관리하는 것 또한 필수적이다. 재발을 방지하는 측면에서 항응고요법은 무엇보다 중요하다. 최근 2016년 ACCP(American College of Chest Physician) 진료지침[1]에서는 심부정맥혈전증의 장기 항응고요법제로 Vitamin K 길항 항응고제(vitamin K antagonists, VKAs)보다는 dabigatran, ribaroxaban, apixaban, edoxaban을 권고하고 있다(Grade 2B). 2014년 유럽심장학회(European Society of Cardiology, ESC) 진료지침[2]은 급성기 항응고요법으로 통상 3개월 이상 권고하고 있고, 초기 5–10일간 미분획 heparin 혹은 저분자량 heparin을 정주하는 항응고요법을 하고 이후 VKA를 병행한 후 유지하는 것을 권고한다. 이 때 dabigatran 혹은 edoxaban 치료는 heparin 정주 치료 후에 시행할 수 있다. Rivaroxaban 또는 apixaban은 heparin 치료와 동시에 혹은 치료 개시 1–2일 후부터 사용하여야 한다. Rivaroxaban은 초기 3주 동안은 15 mg을 1일 2회 사용하고 이후 20 mg을 1일 1회 사용한다. Apixaban은 초기 7일 동안은 10 mg을 1일 2회 사용하고 이후 5 mg을 1일 2회 사용한다. 이처럼 심부정맥혈전증에 대한 항응고요법은 기존의 VKA 치료에서 NOAC (novel or non-vitamin K-dependent new oral anticoagulants)으로 전환되고 있다.

NOAC을 사용한 급성기 항응고요법

폐색전증 또는 정맥혈전색전증에서 NOAC을 사용한 급성기 치료의 3상 임상 시험 결과를 Table 1에 정리하였다. RE-COVER 연구(Efficacy and Safety of Dabigatran Compared to Warfarin for 6 Month Treatment of Acute Symptomatic Venous Thromboembolism)는 정맥혈전색전증의 치료에서 dabigatran과 warfarin을 비교하였다[3]. 1차 분석 지표는 6개월 시점에 평가한 증상을 동반한 정맥혈전색전증의 재발 유무였으며, 2,539명의 환자가 등록되었다. 21% 환자는 폐색전증만 가지고 있었고, 9.6%는 폐색전증과 심부정맥혈전증을 동반하고 있었다. 정주 항응고요법은 약 10일간 시행되었다. 효과 판정 지표 측면에서 dabigatran은 warfarin에 비열등성을 보여주었다(hazard ratio [HR], 1.10; 95% confidence interval [CI], 0.65–1.84). 주요 출혈 발생에서는 통계학적 차이를 보이지 않았으나 전체 출혈 발생의 분석에서는 dabigatran 치료군에서 더 적게 발생함이 확인되었다(HR, 0.71; 95% CI, 0.59–0.85). 2,589명의 환자가 등록한 RE-COVER II 연구[4]에서는 6개월 시점에 평가된 증상을 동반한 정맥혈전색전증의 재발 효과 판정 지표의 측면과 주요 출혈 발생 모두에서 dabigatran이 warfarin에 비열등성을 보여주었다(HR, 1.08; 95% CI, 0.64–1.80). RE-COVER 연구에 등록된 환자들에서 혈전색전증 발생의 HR은 1.09 (95% CI 0.76–1.57)였고, 주요 출혈 발생의 HR은 0.73 (95% CI, 0.48–1.11)이었다.
EINSTEIN-DVT와 EINSTEIN-PE 연구(Oral Direct Factor Xa Inhibitor Rivaroxaban in Patients With Acute Symptomatic DVT/PE)는[5,6] 정맥혈전색전증 환자에서 rivaroxaban (20 mg 1일 1회 투여 이후 15 mg 1일 2회 3주간 투여)과 enoxaparin/warfarin의 비열등성을 비교하였다. EINSTEIN-PE 연구는 증상이 있는 급성 폐색전증 환자 4,832명을 등록한 점이 주목할 만하다. 결과적으로 rivaroxaban은 정맥혈전색전증의 재발 방지에서 기존 치료와 비교하여 열등하지 않음을 보여주었다(HR, 1.12; 95% CI, 0.75–1.68). 또한 경미한 출혈을 포함한 전체 출혈 발생 측면에서는 기존 치료와 차이가 없었다(HR for rivaroxaban, 0.90; 95% CI, 0.76–1.07). 그러나 주요 출혈 발생은 rivaroxaban 투여군에서 더 적게 나타났다(1.1% vs. 2.2%, HR, 0.49; 95% CI, 0.31–0.79).
AMPLIFY 연구(Apixaban for the Initial Management of Pulmonary Embolism and Deep-Vein Thrombosis as First-line Therapy)는 5,395명의 급성 정맥혈전색전증 환자를 대상으로 apixaban (5 mg 1일 1회 투여 후 10 mg 1일 2회 7일간)과 enoxaparin/warfarin을 사용한 기존 치료를 비교하였다. 5,395명의 환자에는 1,836명의 폐색전증 환자가 포함되었다[7]. 1차 유효성 평가 항목은 증상을 동반한 정맥혈전색전증의 재발 혹은 이에 연관된 사망이었다. 안전성 평가 항목은 주요 출혈 발생, 이를 포함한 전체 출혈 발생 두 가지로 평가하였다. Apixaban은 1차 유효성 평가 항목에서 기존 치료에 비하여 열등하지 않음이 확인되었다(relative risk [RR], 0.84; 95% CI, 0.60–1.18). 주요 출혈 발생은 기존 치료에 비해 우월함을 보여주었고(RR, 0.31; 95% CI, 0.17–0.55; p<0.001 for superiority), 전체 출혈도 기존 치료에 비해 더 적게 발생하였다(RR, 0.44; 95% CI, 0.36–0.55; p<0.001).
Hokusai-VTE 연구(Comparative Investigation of Low Molecular Weight (LMW) Heparin/Edoxaban Tosylate (DU176b) Versus (LMW) Heparin/Warfarin in the Treatment of Symptomatic Deep-Vein Blood Clots and/or Lung Blood Clots)는 초기 5일 이상 heparin 치료를 받은 8,240명의 급성 정맥혈전색전증 환자를 대상으로 edoxaban(60 mg 1일 1회 투여, 단 creatinine 청소율이 30–50 mL/min이거나 체중이 60 kg 미만인 경우 30 mg 1일 1회 투여)과 warfarin을 비교하였다[8]. 8,240명의 환자에는 3,319명의 폐색전증 환자가 포함되었다. 연구 약물은 3–12개월 동안 복용하였고, 12개월 간 추적조사를 하였다. Edoxaban은 1차 유효성 평가지표(증상을 동반한 정맥혈전색전증의 재발 혹은 치명적 폐색전증)에서 warfarin에 비해 열등하지 않음이 확인되었다(HR, 0.89; 95% CI, 0.70–1.13). 전체 출혈 발생은 warfarin에 비해 우월함이 확인되었다(HR, 0.81; 95% CI, 0.71–0.94; p=0.004 for superiority). 특히 NT-proBNP (N-terminal of the prohormone brain natriuretic peptide)가 높은(≥ 500 pg/mL) 938명의 급성 폐색전증 환자에서 edoxaban 투여군의 정맥혈전색전증 발생률이 더 낮았다(3.3% vs. 6.2%, HR, 0.52; 95% CI, 0.28–0.98).

NOAC을 사용한 장기 항응고요법

정맥혈전색전증 장기 치료로 dabigatran과 rivaroxaban, apixaban이 연구되어 왔다. 모든 연구에서 폐색전증 환자는 약 1/3을 차지하였고, 나머지 2/3 환자는 폐색전증이 없는 정맥혈전 환자였다. 장기 항응고요법에 대한 연구들의 개요와 결과를 Table 2에 정리하였다.
Dabigatran은 두 개의 연구에서 warfarin 혹은 위약과 비교되었다. RE-MEDY 연구(Secondary Prevention of Venous Thrombo Embolism)에서 2,866명의 환자들이 dabigatran (150 mg 1일 2회 투여) 혹은 warfarin(international normalized ratio [INR] 2–3) 군에 무작위 배정되었고, dabigatran은 증상을 동반한 정맥혈전색전증의 발생 혹은 이로 인한 사망 지표에서 warfarin에 비하여 열등하지 않음이 확인되었다(HR, 1.44; 95% CI, 0.78–2.64; p=0.01 for non-inferiority)[9]. Dabigatran 투여군의 주요 출혈은 0.9%였고 warfarin 투여군은 1.8%였다(HR, 0.52; 95% CI, 0.27–1.02). RE-SONATE 연구(Twice-daily Oral Direct Thrombin Inhibitor Dabigatran Etexilate in the Long Term Prevention of Recurrent Symptomatic VTE)에서 1,353명의 환자는 6개월 동안 dabigatran과 위약 투여군에 무작위 배정되었고, dabigatran은 증상을 동반한 정맥혈전색전증의 재발과 설명되지 않는 사망의 감소 측면에서 92%의 위험률 감소 효과를 보여주었다(HR, 0.08; 95% CI, 0.02–0.25). 주요 출혈은 dabigatran 군은 0.3%, 위약군에서는 0% 발생하였고, 전체 출혈은 dabigatran군에서 5.3%, 위약군에서는 1.8% 발생하였다(HR, 2.92; 95% CI, 1.52–5.60).
EINSTEIN Extension 연구는 무작위 이중맹검으로 진행되었고, 정맥혈전색전증의 장기 치료에서 rivaroxaban의 효능과 안전성을 평가하였다[5]. 6–12개월의 1차 치료를 마친 환자에서 추가적인 6–12개월의 rivaroxaban 치료를 위약과 비교하였다. Rivaroxaban은 정맥혈전색전증의 재발에서 위약보다 우월함을 보여주었다(1.3% vs. 7.1%; HR, 0.18; 95% CI, 0.09–0.39). Rivaroxaban 군에서 치명적이지 않은 주요 출혈은 0.7%에서 발생하였고, 전체 출혈은 6%에서 발생하였다(HR, 5.19; 95% CI, 2.3–11.7).
AMPLIFY Extension연구[10]는 정맥혈전색전증 환자를 apixaban (2.5 mg 또는 5 mg 1일 2회 투여)과 위약 투여군에 12개월간 무작위 배정하였다. 증상을 동반한 정맥혈전색전증의 재발은 위약군에서는 11.6%, apixaban 2.5 mg군에서는 3.8% (HR, 0.33 vs. placebo; 95% CI, 0.22–0.48), apixaban 5 mg 군에서는 4.2% (HR, 0.36 vs. placebo; 95% CI, 0.25–0.53) 발생하였다. 주요 출혈은 위약군에서는 0.5%, apixaban 2.5 mg 군에서는 0.2%, apixaban 5 mg 군에서는 0.1% 발생하였다. 전체 출혈은 위약군에서는 2.7%, apixaban 2.5 mg 군에서는 3.2% (HR, 1.20 vs. placebo; 95% CI, 0.69–2.10), apixaban 5 mg 군에서는 4.3% (HR, 1.62 vs. placebo; 95% CI, 0.96–2.73) 발생하였다.

결론

지금까지 연구들은 정맥혈전색전증의 급성기 치료에 NOAC의 사용이 효과 면에서 기존 heparin과 VKA 치료에 비해 열등하지 않고, 주요 출혈 발생 측면에서 보다 안전할 수 있음을 보여주었다. NOAC을 사용한 정맥혈전색전증의 장기 항응고요법의 효과 판정은 급성기 치료 효과 판정의 연장선상에 있다. 이에 대한 연구 결과들은 NOAC의 장기 치료가 증상을 동반한 정맥혈전색전증의 재발에 효과적이고 주요 출혈 발생에 기존 VKA보다 안전할 수 있음을 보여주었다. 이 연구들에서 TTR (time to therapeutic INR range)이 비교적 높았다는 점은 고무적이나, 상대적으로 젊은 환자가 많았고 암환자가 아주 드물었다는 점은 숙고해야 할 점이다. 현재 NOAC은 기존 치료를 대체하고 있으나 아직도 경험이 부족한 상태이며, 많은 임상 상황에서 경험 축적이 필요하다.
