아시아인에서 NOAC의 효과

Abstract

Anticoagulation treatment, including novel or non-vitamin K-dependent antagonist oral anticoagulants (NOACs), is essential to prevent thromboembolic events in high-risk atrial fibrillation patients. There are not enough studies on the effect and safety of NOACs in Asians. Due to the low body surface area, genetic polymorphism, and herbal diet of Asians, it is difficult to attain optimal anticoagulation with traditional anticoagulation treatment using vitamin K antagonists, and more bleeding complications are reported with this treatment. In several recent studies, the use of NOACs in Asians resulted in lower thrombo- embolic events and fewer bleeding complications than those with a vitamin K antagonist. Given the race-related differences of Asians, NOACs have sufficient efficacy and safety for the prevention of thromboembolic events in patients with atrial fibrillation.

서론

심방세동으로 인한 뇌졸중 및 전신 색전증의 위험과 이를 방지하기 위한 warfarin의 효과는 잘 알려져 있다[1,2]. 하지만 warfarin은 다른 음식이나 약물과 반응을 보이기 때문에 약효를 유지하기 위하여 정기적인 혈액검사를 통한 관찰이 필요하다[3]. 이에 반해 새로운 경구 항응고제(novel or non-vitamin K-dependent antagonist oral anticoagulant, NOAC)는 혈액검사를 통한 모니터링이 필요 없어 비판막성 심방세동 환자의 혈전색전증 예방을 위해 많이 사용되고 있다.
심방세동의 유병률은 서양인에서 1-2%, 동양인에서 1%정도로 알려져 있지만 나이가 듦에 따라 심방세동의 유병률이 증가하고 고령 인구가 많은 동양의 특성상 심방세동 환자의 절대 수는 미국이나 유럽보다 동양에서 더 많다[4]. 하지만 현재까지 NOAC의 효과와 안전성에 대한 연구는 서양인과 관련된 것이 대부분이며, 동양인에서의 연구는 많지 않은 실정이다. 이에 대해 저자는 그 동안 발표된 연구를 바탕으로 동양인에서의 NOAC에 대해 전반적인 고찰을 해보려 한다.

NOAC

비판막성 심방세동에 대해 기존 warfarin을 대체할 수 있는 NOAC에 대한 몇몇 연구가 시행되어 왔다[5-8]. 직접 트롬빈 억제제(direct thrombin inhibitor)인 dabigatran과 응고인자 Xa 억제제(factor Xa inhibitor)인 rivaroxaban, apixaban, edoxaban은 기존 warfarin과 비교하여 뇌졸중, 전신 색전증, 주요 출혈 등의 측면에서 우월하거나 동등한 효과를 나타냈다(Table 1)[6-8]. 이들 연구로 인해 NOAC이 비판막성 심방세동의 뇌졸중 및 색전증 예방의 표준 치료로 자리 잡게 되었다.

아시아인에서 NOAC의 효과와 안전성에 대한 연구

아시아인에서 NOAC의 효과를 직접 관찰한 연구는 거의 없고, 대부분 기존의 글로벌 연구에서 아시아인만을 하위 분석한 것으로서 dabigatran을 제외하고는 아시아 중에서도 동아시아인을 대상으로 한 결과이다[9-12].
아시아인에서 NOAC의 효과는 약제마다 다소 다른 결과를 보이지만, 대부분 비아시아인에 비해 좋은 결과를 보인다(Figure 1). 뇌졸중/색전증 예방 측면에서 dabigatran 150 mg 1일 2회 투여는 통계적으로 유의한 감소를 보였고(hazard ratio [HR], 0.45; 95% confidence interval [CI], 0.28-0.72) dabigatran 110 mg 1일 2회 투여, rivaroxaban, apixaban, edoxaban 60 mg 투여의 경우는 통계적 유의성은 없었지만 뇌졸중/색전증이 줄어드는 경향을 보였다. 허혈성 뇌졸중을 종료점으로 하였을 때에도 비슷한 결과를 보였는데, dabigatran 150 mg 1일 2회 투여 군에서만 통계적으로 유의하게 감소하는 결과를 보였다(HR 0.55, 95% CI 0.32-0.95). Edoxaban 60 mg 투여군은 warfarin 군에 비해 총사망률이 감소하는 결과를 보였다.
안전성 문제에 관해서 무엇보다 고무적인 것은 항응고제의 치명적인 부작용인 출혈성 뇌졸중에 대한 결과인데, rivaroxaban 20 mg 군을 제외한 모든 NOAC 군에서 통계적으로 유의하게 출혈성 뇌졸중 발생이 감소하였다. 주요 출혈 사건은 rivaroxaban을 제외한 NOAC에서 warfarin에 비해 유의하게 적게 발생하였다.

결론

이전의 연구 결과를 보면 서양인에 비해 동양인에서 출혈성 뇌졸중이 더 잘 발생하고[13], 뇌졸중으로 혈전 용해제를 투여한 경우 출혈성 합병증 발생률이 높았으며[14], 이에 따라 아시아에서는 상대적으로 낮은 INR (international normalized ratio)을 유지하려는 경향이 있었다[15]. 그 원인은 적은 체중과 체표면적, warfarin 대사 효소의 유전적 다형성(polymorphism), vitamin K를 함유한 식물 섭취 등이다[14,16]. 하지만 NOAC은 이러한 warfarin의 단점을 극복하게 해주었다.
이러한 효과를 확인하기 위해서는 아시아인만을 대상으로 한 연구뿐 아니라, 국가, 민족별 NOAC의 효과 차이에 대한 연구가 필요할 것으로 생각된다.
