급성관동맥증후군을 동반한 심방세동 환자에서 NOAC 치료

Abstract

Atrial fibrillation (AF) can occur in acute coronary syndrome (ACS), which is a serious medical condition and may require the use of antiplatelet agents in addition to anticoagulants for stroke prevention. Recently, novel or non–vitamin K antagonist oral anticoagulants (NOACs) have been increasingly used for stroke prevention in patients with AF instead of traditional OACs. The duration of treatment or treatment with a stepwise approach (e.g. triple, double, or monotherapy) is determined depending on the clinical setting and the balance between the risks of ischemic stroke and bleeding. However, some concerns and controversies in the use of NOACs in patients with AF and ACS need to be addressed. Here, the current management for NOAC therapy in patients with ACS and AF will be reviewed based on recently published guidelines.

서론

심방세동 환자의 6-20%는 경피적 관동맥중재술(percutaneous coronary intervention, PCI)을 경험하게 된다고 알려져 있다[1-3]. 특히 급성관동맥증후군(acute coronary syndrome, ACS)이 발생했을 경우 심방세동에서의 뇌졸중 예방을 위해 항응고요법이 필요한 동시에 PCI 후 항혈소판제의 투여도 필요하므로 이로 인한 출혈 등 여러 문제가 발생할 수 있다. 최근 심방세동 환자에서 뇌졸중 예방을 위해 비-비타민 K 길항제 경구 항응고제(non-vitamin K antagonist oral anticoagulant, NOAC) 사용이 증가하고 있으며, ACS에서 NOAC과 항혈소판제 병합에 따른 출혈 위험성 때문에 병용하는 항혈소판제의 종류, 개수 및 병합 기간 등을 결정하는 데 어려움이 있을 수 있다. 이에 최근 발표된 유럽심장학회(ESC) 진료지침과 관련 연구들을 중심으로 ACS를 동반한 심방세동 환자에서 NOAC과 항혈소판제의 치료에 대해 알아보고자 한다[4-12].

ACS에서 Vitamin K 길항 항응고제와 항혈소판제 병용 치료

2014년 유럽심장학회, 유럽부정맥학회(EHRA), 유럽경피적심혈관중재술학회(EAPCI), 급성심혈관관리학회(ACCA)가 함께 ACS 또는 안정형 협심증에서 PCI를 시행받은 비판막성 심방세동 환자에서 NOAC을 포함한 항혈전 치료 전략에 대한 진료지침을 발표하였다[4]. 이제항혈소판요법(dual antiplatelet therapy, DAPT)을 받는 ACS 환자에서 심방세동이 발생했을 경우 CHA2DS2-VASc 점수에 따라 VKA (vitamin K antagonist) 또는 NOAC 등을 투여할 것을 권고하고 있으나, DAPT를 함께 하는 삼제요법(triple therapy)의 경우 출혈 발생 위험이 높아져 환자의 뇌졸중 위험도와 출혈 위험도를 함께 고려할 것을 권고하였다.
항응고 치료 방법은 뇌졸중 위험도, 출혈 위험도, 임상적 상황 및 항응고요법의 4가지 사항을 고려하여 선택할 수 있다. 뇌졸중 위험도가 비교적 낮은 CHA2DS2-VASc 1점인 경우 출혈 위험도가 낮은 HAS-BLED 0-2에서는 6개월간 삼제요법으로 치료하고 6개월 후에는 이제요법(dual therapy; OAC+aspirin 또는 clopidogrel), 출혈 위험도가 높은 HAS-BLED ≥3에서는 4주 동안 삼제 또는 이제요법으로 치료하고 4주 이후에는 이제요법을 유지하며, 12개월 이후에는 두 가지 모두 OAC 단일요법(monotherapy) 유지를 권고하고 있다. 뇌졸중 위험이 높은 CHA2DS2-VASc ≥2인 경우 HAS-BLED 0-2에서는 6개월 간 삼제요법 후 6개월간 삼제 또는 이제요법, HAS-BLED ≥3이라면 4주간 이제 또는 삼제요법 후 이제요법을 권고한다. 두 경우 동일하게 12개월 후에는 OAC 단일요법을 권고한다.

ACS에서 NOAC과 항혈소판제 병용요법

Oldgren J 등은 RE-DEEM 연구에서 DAPT 환자군에 비해 dabigatran을 포함한 삼제요법군에서 최근 발생한 심근경색 환자에서의 응고 활성도를 감소시킴과 동시에 용량의존적으로 출혈 위험이 증가한다고 하였다[8]. Mega JL 등은 ATLAS ACS-TIMI 46 연구에서 APT 환자군에 비해 rivaroxaban을 추가한 환자군에서 사망, 심근경색, 뇌졸중은 감소하였으나 출혈 발생 위험은 높다고 하였다[9]. ATLAS ACS-TIMI 51 연구에서는 rivaroxaban 2.5 mg 1일 2회 투여군에서 5 mg 1일 2회 투여군에 비해 출혈 위험이 감소한다고 보고하였다[10]. Alexander JH 등이 보고한 APPRAISE 연구에서 항혈소판제에 추가하여 apixaban을 투여했을 경우 허혈성 합병증 감소 경향이 있으나 용량 의존적으로 출혈이 증가함을 보고하였다[11]. 종합해 보면 삼제 또는 이제요법 시 NOAC을 사용할 경우 출혈 위험이 높다는 결과들을 보여주고 있어 이에 대한 주의를 강조하고 있다.

ACS를 동반한 심방세동 환자에서 NOAC 치료

최근 ESC와 EHRA에서 발표한 가이드라인에서는 NOAC으로 치료받고 있는 심방세동 환자에서 ACS가 발생한 경우의 치료지침을 제시하고 있다(Figure 1)[5,6]. 입원 시 우선 NOAC 투여를 중지하고 aspirin과 P2Y12 억제를 투여한다. 이후 ST분절상승심근경색(ST-segment elevation myocardial infarction, STEMI)의 경우 PCI와 섬유소 용해(fibrinolysis) 두 가지 치료로 구분하는데, PCI 시 요골동맥 접근(radial access)과 신세대(new generation) 약물방출스텐트의 사용을 권고하고 있다. 비ST분절상승심근경색(non ST-segment elevation myocardial infarction, NSTEMI)의 경우 비응급과 응급으로 구분하여 치료 전략을 제시하고 있다. Heparin 등 항응고 주사제 투여를 중지한 후 APT에 추가하여 이전과 동일한 종류의 NOAC을 재투여하는데, dabigatran 150 mg 1일 2회 경우 110 mg 1일 2회로 감량을 고려할 수 있으며, factor Xa 억제제의 경우 출혈 감소를 위해 rivaroxaban 15 mg 1일 1회, apixaban 2.5 mg 1일 2회, edoxaban 30 mg 1일 1회의 저용량 투여를 고려할 수 있으나 뇌졸중 예방에 대한 근거는 아직까지 부족한 상황이다. NOAC과 항혈소판제를 동시에 투여하는 모든 환자에서는 양성자 펌프 억제제 투여를 고려해야 하며, 장기 치료(Figure 2)를 계획한 후 퇴원시킨다[6]. 퇴원 이후 삼제요법(NOAC+aspirin+clopidogrel) 6개월 후 6개월간 이제요법(NOAC+apirin 또는 clopidogrel)을 하고, 12개월 이후 NOAC 단일요법으로 유지할 것을 권고하고 있으며, 출혈 위험도와 죽상경화혈전 위험도(atherothrombotic risk) 등에 따라 투여 기간을 조정할 수 있다.
심방세동 환자에서 NOAC 치료 시 서로간의 직접 비교 연구가 부족하듯이 ACS를 동반한 심방세동 환자에서 NOAC의 종류 선택에 대한 근거는 부족하다. 2015년 ESC 가이드라인에서 ALAS ACS-TIMI 51 연구를 근거로 rivaroxaban 2.5 mg 1일 2회 투여가 삼제요법 방법으로 제시되었으나, 이전에 뇌졸중이나 일과성 뇌허혈발작(transient ischemic attack)이 없는 환자로 대상을 제한하고 있다[10].

향후 전망

현재까지 경구 VKA와 NOAC에 대한 대규모 전향적 비교 자료는 부족한 상태이다. 첫 무작위 연구인 PIONEER AF-PCI 연구(NCT01830543)는 PCI 시행 후 APT를 받는 비판막성 심방세동 환자에서 VKA 투여 환자군과 rivaroxaban 투여 환자군에서의 출혈 합병증 등을 비교하는 연구로 현재 진행 중이다[13]. DAPT와 병용하는 삼제요법 시 dabigatran과 warfarin을 비교하는 RE-DUAL PCI 연구(NCT02164864)도 진행 중이다.

결론

ACS를 동반한 심방세동 환자에서 APT와 NOAC의 병용 투여 치료 전략은 환자의 뇌졸중 위험도(CHA2DS2-VASc), 출혈 위험도(HAS-BLED) 및 임상적 상황과 치료 전략 등에 따라 결정된다. 급성기에는 삼제요법을 하고 6개월 이후에는 이제요법으로 전환한 후 12개월부터는 NOAC 단일요법이 제시되고 있다. 현재 진행중인 대규모 연구들의 결과가 향후 치료 방침 결정에 도움을 줄 것으로 기대한다.
