2018 대한부정맥학회 심실빈맥 전극도자 절제술 가이드라인 Part 2

Abstract

The recommendations outlined constitute the first clinical practice guidelines of the Korean Heart Rhythm Society regarding catheter ablation of ventricular arrhythmias (VA). This is a guideline PART 2, which includes VA in the structurally normal heart, inherited primary arrhythmia syndromes, VA related to congenital heart disease, as well as VA and sudden cardiac death observed in specific populations. In the structurally normal heart, treatment is guided by the occurrence of symptoms or the frequency of arrhythmias that cause ventricular dysfunction over time. Catheter ablation can be recommended in patients in whom anti-arrhythmic medications are inef-fective. The sites of origin of arrhythmic activity are known to be the outflow tract, fascicles, papillary muscle, or the annulus. Specific cardiac channelopathies include congenital long QT and Brugada syndrome. This guideline discusses the diagnostic criteria, risk stratification, and treatment of these syndromes. We have included recommendations for adult congenital heart disease. Moreover, we have discussed the management of VA occurring in specific populations such as in patients with psychiatric and neurological disorders, pregnant patients, those with obstructive sleep apnea or drug-relat-ed pro-arrhythmias, athletes, and elderly patients.

1. 구조적 심질환이 없는 환자에서의 심실빈맥

1.1 유출로 심실빈맥 (Outflow tract ventricular tachycardia)

특발성 심실성 부정맥이 기시하는 가장 흔한 부위는 심실 유출로이며, 그중 70% 는 우심실 유출로에서 기인한다.4,9 그 이외에, Valsalva 동, 좌심실 유출로, 심외막, 심장대정맥 혹은 드물게 폐동맥에서 기시하는 경우도 있다.1015 이 부위들의 해부학적 근접성으로 인해, 심전도를 통한 정확한 기시부 감별은 어렵지만, 일반적으로 우심실유출로 심실빈맥은 전형적인 하부축과 우세한 좌각차단 형태를 보인다.4 대부분의 특발성 심실성 부정맥은 방아쇠 작용이나 이상 자동능을 기전으로 하는 국소 기전을 바탕으로 일어난다.16 증상은 다양하게 나타날 수 있으며, 구조 심질환 여부를 감별하는 것이 중요하다. 심기능 저하를 유발할 만한 빈번한 부정맥이 발생하는 경우에는 주기적인 심기능 평가가 필요하다.17,18 증상이 없거나 경미한 경우에는 과도한 카페인 섭취 제한 혹은 교감신경 흥분제 제한 등의 보존적 요법으로 충분하지만, 증상이 있는 경우에는 베타 차단제, 혹은 비 디히드로피리딘계 칼슘통로길항제 치료가 필요하며, 소디움 통로 차단제(Class Ic)도 도움이 될 수 있다.68 이러한 항부정맥제 치료에 효과가 없거나, 약을 복용할 수 없는 경우에는 카테터 절제술을 해야 한다.1,2,4
카테터 절제술 원칙은 전기 활성이 가장 빠른 곳을 찾거나 (activation mapping), 이것이 불가능한 경우에는 pace-mapping을 통해 기시부를 찾을 수 있다.12,19 유출로 부위 심실성 부정맥의 기시부는 12 유도 심전도를 통해 예측해 볼 수 있는데, 이를 통해 카테터의 접근 방법이나 성공율, 위험도 등을 예측해 볼 수 있다.12,13
우심실 유출로 기원의 심실부정맥은 대부분 폐동맥판 아래부위에서 발생한다.11,20 좌심실 유출로 기원의 심실부정맥은 대동맥 첨두, 대동맥판 아래부위, aorto-mitral continuity, 혹은 좌심실 외막의 summit 에서 발생할 수 있다.14,15,21,22 이외에도 심장대정맥이나 심외막에서 발생하는 경우도 있는데, 관상동맥과 근접하여 시술이 불가능한 경우도 있다.23,24 좌심실 유출로는 해부학적으로 복잡하여 정확한 지도화와 절제술을 위해 카테터 접근을 경중격 그리고 역행성 방법을 함께 이용하는 것이 요구된다.22 Valsalva 동에서 기원하는 심실성 부정맥은 좌관첨판에서 가장 흔히 발생한다. 좌관첨판에서 시술 할 때, 좌주간부 관동맥 손상을 주의해야 하는데, 적어도 6mm 이상의 거리가 필요하다. 관상동맥 손상을 확인 위해 관상동맥 조영술이 필요할 수 있다.

1.2 베라파밀 민감성 섬유속 심실빈맥 (Verapamil sensitive fascicular ventricular tachycardia)

특발성 좌심실 빈맥의 가장 흔한 종류는 좌후 섬유속 심실빈맥 (left posterior fascicular ventricular tachycardia) 으로 특발성 좌심실 빈맥의 90% 이상을 차지한다. 주로 15세에서 40세 정도의 젊은 연령층에서 나타나며 남자에게 더 흔하게 발생한다. 심전도에서는 우각차단 형태에 상향축 (superior axis) 모양을 보인다. 이 빈맥은 비정상적인 지연 전도를 나타내는 Purkinje 조직을 통한 앞 방향 전도와 후섬유속을 통한 역방향 전도로 이루어지는 회귀 기전을 통해 발생한다. 지연 전도를 보이는 Purkinje 조직이 베라파밀에 민감한 부위이며 베라파밀 사용을 통해 빈맥을 종료시킬 수 있어 베라파밀 민감성 섬유속 심실빈맥이라고도 부른다.13
드물게 나타나는 하향축을 보이는 좌전 섬유속 심실빈맥 (left anterior fascicular ventricular tachycardia) 과 더 좁은 QRS 복합체와 정상축을 보이는 좌상 중격 섬유속 심실빈맥 (left upper septal fascicular ventricular tachycardia) 도 있으며 이는 특발성 좌심실 빈맥에서 10% 미만, 1% 미만의 비율을 차지한다.4,5
급성기 치료로서 베라파밀 정맥주사는 베라파밀 민감성 심실빈맥의 치료에 효과적이나 베라파밀 경구 복용에 의한 장기적 치료효과는 잘 알려져 있지 않다. 젊은 나이에 나타나는 경우가 많으며 장기간 베라파밀 사용의 효과가 제한적이기 때문에 특발성 좌심실빈맥, 특히 섬유속 심실빈맥에서는 카테터 절제술을 일차 치료로 권고한다.1,2,4,6,7
카테터 절제술의 목표는 앞 방향 전도를 담당하는 Purkinje 조직의 가장 말단 부위이며 좌후 섬유속 심실빈맥의 경우에는 좌후 섬유속과 인접하는 좌심실의 중격이 이 부위에 해당된다. 좌후 섬유속 심실빈맥에 대한 카테터 절제술의 성공률은 90% 이상으로 보고되었으며 재발률은 약 10% 정도로 보고되었다.

1.3 유두근 심실빈맥 (Papillary muscle ventricular tachycardia)

특발성 심실빈맥이나 심실조기수축이 우심실이나 좌심실의 유두근에서 발생할 수 있다.15 국소성, 비회귀성 기전으로 비지속성 심실빈맥이나 심실조기수축의 형태로 흔히 나타나며 운동과 연관되어 나타나는 경우가 많아 에피네프린 이나 아이소프로테레놀 (isoproterenol) 정주로 유도될 수 있다.1 좌후 유두근 (left posterior papillary muscle) 에서 발생할 경우에는 우각차단 형태와 우상 혹은 좌상축을 보이며 150ms 이상의 QRS 지속시간을 가진다.1 증상이 있는 유두근 심실빈맥 환자에서는 베타차단제, Class Ic 항부정맥제의 사용을 권고하고 약제에 반응하지 않는 경우에는 카테터 절제술이 효과적인 치료방법이 될 수 있다. 하지만 카테터 절제술 시술 시 유두근 부위에서 카테터의 안정성이 불안정할 경우가 많기 때문에 심방중격 천자를 통한 접근과 심장내 초음파 유도 하에 카테터 절제술을 시행하는 것을 권고한다.5 이런 이유로 시술 성공률은 높으나 재발률은 다른 종류, 특히 특발성 유출로 심실빈맥에 비하여 높다. 드물지만 발생 가능한 심한 합병증으로는 판막 손상이 있을 수 있다.

1.4. 판륜 심실빈맥 (Annular ventricular tachycardia)

승모판륜 심실빈맥은 특발성 심실부정맥의 5%를 차지하는 것으로 보고되었다.1 QRS 복합체는 우각차단 형태에 V6 유도에 지속되는 S파를 보인다. 삼천판륜 심실빈맥은 특발성 심실 부정맥의 8% 정도를 차지하며 QRS 복합체는 좌각차단 형태와 좌축편위 (left axis deviation) 을 가진다.2 Class Ic 항부정맥제나 베타차단제 사용에 반응하지 않는 경우에는 카테터 절제술이 효과적인 치료방법이 될 수 있다.

1.5 특발성 심실세동 (Idiopathic ventricular fibrillation)

특발성 심실세동은 구조적 심질환이 없고 급사를 유발할 수 있는 다른 유전적 부정맥 질환이 없을 때 진단할 수 있다. 베타차단제나 Class III 항부정맥제에 의해 그 발생을 줄일 수 있으나 완전히 예방할 수는 없으며, 심실세동의 재발 위험도가 매우 높기 때문에 급사 예방을 위한 삽입형 제세동기 시술이 권고된다.1
특발성 심실세동은 주로 우심실 혹은 좌심실의 Purkinje 계에서 발생하는 심실조기수축에 의해 유발되며 우심실 유출로 심실조기수축에 의해서 유발되는 경우도 있다.25 반복적인 심실세동이 발생하는 경우에는 심실세동을 유발시키는 심실조기수축 제거를 목표로 시행하는 카테터 절제술이 권고된다. Purkinje 계에서 발생하는 심실조기 수축에 대한 카테터 절제술 시에는 심실조기수축에 선행하는 고주파의 Purkinje 전위 (high-frequency Purkinje potential) 가 제거 목표가 된다. 심실유출로 심실조기수축의 경우 심실조기수축시 가장 빠른 심실 활성화를 보이는 부위가 제거 목표가 된다. 심실조기수축이 자주 나타날수록 도자절제술의 성공률은 높아진다. 그러나, 원인 심실조기 수축의 성공적인 제거 후에도 약 10%의 환자에서 재발이 보고되고 있으며, 5년이상 추적 관찰 연구에서 18%의 심실세동 혹은 급사의 발생을 보고한 바 카테터 절제술의 성공 유무와는 상관 없이 삽입형 제세동기의 시술은 반드시 시행되어야 한다. 4,5

2. 유전성 부정맥 증후군 (Inherited primary arrhythmia syndromes)

유전성 부정맥 증후군은 구조적인 심장 이상 소견없이 유전적 요인에 의해 심실성 부정맥이 유발되는 경우로, 긴 QT 증후군 (long QT syndrome), 짧은 QT 증후군 (short QT syndrome), 부루가다 증후군 (Brugada syndrome), 카테콜아민 유발 다형심실빈맥 (catecholaminergic polymorphic ventricular tachycardia), 조기 재분극 증후군 (early repolariazation syndrome) 등이 있다. 본 진료 지침에서는 긴 QT 증후군과 부루가다 증후군에 대해서만 기술하였다.

2.1 긴 QT 증후군

가족력이 없는 무증상 환자에서 긴 QT 증후군을 진단하는 기준으로 제시되는 QTc >500 ms 는 급성 심장사 (sudden cardiac death) 에서 부정맥 발생의 고위험과 연관이 되고, QTc ≥480 ms 또는 LQTS risk score >3은 긴 QT 증후군의 임상적 진단 기준으로 사용되어 왔으며, 원인 불명의 실신 있을 경우에는 QTc ≥460ms 도 긴 QT 증후군으로 진단할 수 있다.14 현재까지 긴 QT 증후군과 연관된 15개 이상의 유전자 (LQTS) 변이가 알려져 있는데, 그 중 3가지 유전자 (KCNQ1, KCNH2, SCN5A)가 양성으로 판명된 환자의 약 90%를 차지하며, 그 유전자 변이형에 따라 임상 예후의 차이를 보이기도 한다.45 유전자 검사상 LQTS 유전자 변이 이상으로 확인된 긴 QT 증후군 환자의 약 20-25% 는 QTc 간격이 정상 범위로 알려져 있으며, 경험이 많은 기관에서 시행한 유전자 검사상 병적인 LQTS 변이 확진 시 QT 간격에 상관없이 긴 QT 증후군으로 진단할 수 있다.6

임상 양상, 심전도 소견 및 유전자 검사 결과 등을 종합적으로 평가하여 환자 개개인에 대한 위험도 평가가 이루어져야 한다.6 생활 습관에 대한 주의 사항이 모든 긴 QT 증후군 환자에서 권고된다. 일차 약제인 베타차단제는 심장 부정맥 발생을 감소시키므로 모든 긴 QT 증후군 환자에서 사용할 것을 권고하나 LQTS 3형의 경우 남자에서의 치료 효과에 대한 보고는 아직까지 제한적이다. 심장마비를 경험한 경우에는 제세동기 삽입을 권고하고 있다. 심장마비를 경험하지 않은 경우에는 임상 양상, 심전도 소견 및 유전자 검사 결과 등을 종합한 환자 개개인에 대한 위험도 평가를 통해 제세동기 삽입 여부를 결정한다.

2.2 부루가다 증후군

부루가다 심전도 양상을 유발하는 약물이나 술, 고열 등은 피해야 한다. 심정지 또는 심실빈맥이 확인된 경우 제세동기 삽입을 권고한다. 심실빈맥 확인이 어려운 경우에는 실신 등의 임상 양상 및 전기생리학검사 등의 위험도 평가를 통해 제세동기 삽입 여부를 고려할 수 있다. 심실세동 발병 전 심실조기수축 결합 간격이 재분극 증후군 (early repolarization syndrome)과 부루가다 증후군에서 차이가 있는 것으로 보고되었으며, 부루가다 증후군 환자에서 발생한 전기 폭풍의 경우 quinidine 또는 isoproterenolol 투여를 고려해야 한다.3 전기 폭풍의 병력이 있거나 심실빈맥으로 인한 반복적인 제세동기 전기충격이 있는 환자에서 심외막 카테터 절제술을 고려할 수 있다.

3. 선천성 심장질환과 연관된 돌연심장사와 심실성 부정맥

선천성 심장질환은 천 명의 신생아당 7~8명의 발생율을 보인다. 지난 수십년 동안 선천성 심장질환에 대한 진단 및 치료법의 발전으로 환자들의 장기 생존이 향상되었다. 적절한 교정 수술 이후 장기 생존한 선천성 심장질환 환자들의 사망 원인은 심부전과 심실빈맥 및 심실세동이었다.3 선천성 심장질환 환자에서 심실빈맥 및 심실세동의 위험인자에 대한 대규모 전향 연구 결과는 아직까지 없지만, 팔로 4 징과 수정대혈관 환자 군에 대한 비교적 대규모 후향적 연구가 발표되었다.1,4 특히, 팔로 4징에 대한 연구 결과가 가장 많은데, 넓은 QRS 폭 (>180 ms), 우심실 과부하, 광범위한 우심실 반흔, 좌심실 기능부전, 심실빈맥 등이 돌연심장사의 위험인자로 알려져 있으며, 위험도 평가를 위해 전기생리학 검사를 고려할 수도 있다.2,5,7 이러 한 고위험군을 대상으로 삽입형 제세동기를 시술하였을 때, 년간 8~10% 정도의 높은 심실빈맥 및 심실세동 발생이 기록되었다는 보고가 있다.4 팔로 4징 환자에서 심실빈맥이 발생하였을 때 카테터 절제술이 효과적일 수 있으며, 심실 빈맥 전기 폭풍, 재발성 제세동기 전기충격이 프로그래밍 조절이나 약제로 치료되지 않을 때 삽입형 제세동기의 보조 치료로서 카테터 절제술이 추천된다.6,12 대혈관 전위의 경우 심실빈맥 뿐 아니라 심방빈맥이 심실세동으로 악화되는 경우도 있어 주의를 요한다.13 단심실 질환에 대한 폰탄 수술 이후에도 심방빈맥 및 그로 인한 심부전이 문제가 되며, 팔로 4징과 비교했을 때 돌연심장사의 위험은 상대적으로 적은 편이다.

4. 특별한 환자 군의 부정맥 위험도

4.1 정신질환

정신분열증 환자의 돌연심장사 위험은 일반인에 비해 3배정도 높다. 일부 항정신병약과 항우울증약은 심실성 부정맥, 돌연심장사와 연관이 있다고 알려져 있으며 이는 Torsades de pointes 와 연관이 있는 것으로 알려져 있다. 따라서 이러한 환자의 심실부정맥을 치료할 때는 각별한 주의가 필요하다. 삼환계 항정신병약물은 SSRI (selective serotonin reuptake inhibitors) 보다 QTc 간격의 연장과 Torsades de pointes 발생과 더 연관이 있다. 소디움 통로 차단제가 부정맥의 병력이 있는 환자, 좌심실 기능부전이 있는 환자, digoxin 복용환자, 저칼륨혈증 환자에서 사용되었을 때 흔히 연관이 있다. 정신질환의 약물치료를 개시한 후에는 심장에 대한 주의 관찰이 필요하고, 심전도상 QTc 간격이 500 ms 이상이거나 심장 증상이 발생하였을 때는 약물 투여를 재고해야 한다. 약물 병합으로 QTc 간격이 증가될 수 있는 약물의 병합은 피해야 한다.

4.2 신경근육계 질환 환자

근이영양증은 골격근과 심근 모두를 침범할 수 있는 유전성 질환이다. 심장 이완은 심근의 섬유화 과정과 지방 침착의 퇴행성 변화의 결과로 발생한다.5 가장 흔한 발현 형태는 확장성 심근병증과 전도계 손상이다. 모든 근이영양증에서 호흡근 침범은 삶에 질적, 양적 영향을 미치게되므로 예방적 장치의 사용이 고려된다.
Duchenne씨 이영양증, Becker씨 이영양증, Steinert씨 병, Emery–Dreifuss 근이영양증, 제1형 지대근 이영양증 환자에서 심장 확장이 흔히 나타난다. 확장성 심근병증 발생시 심실조기수축과 비지속성 심실빈맥, 전도계 장애가 나타날 수 있기때문에 부정맥 치료는 심근병증 치료와 병행되어야 한다. 삽입형 제세동기의 예방적 사용은 비허혈성 확장성 심근병증에서의 치료와 같은 맥락으로 적용되어야 한다.5

임신은 구조 심질환이 있는 여성에서 상당한 위험을 초래한다.1 선천성 긴 QT증후군이 있는 여성에서 임신은 출산 후 심장사건발생 가능성을 증가시킨다. 따라서 베타차단제의 지속적인 사용이 필요하다. 브루가다 증후군이 있는 여성은 안전하게 임신 및 주산기를 가질 수 있다.
경증 부정맥이 발견되었을 때는 환자를 안정시켜야 하고 흡연 음주 카페인을 금지시켜야 한다. 계획된 임신의 경우에는 증상이 있는 빈맥성 부정맥은 임신 전 카테터 절제술로 제거해야 한다. 약물 치료를 할 계획이라면, 가능하면 임신 후반기에 시작하는 것이 좋겠고 저용량을 사용할 수 있다. 구조적 심질환이 없는 임신중 부정맥은 베타차단제에 대부분 잘 반응한다.1,5,6 베타차단제에 반응하지 않을 경우 소타롤이나 소디움 통로 차단제를 고려할 수 있다. 임신 초기의 약물 사용은 태아 기형과 연관이 있고, 임신 후기 약물 노출은 부정맥 유발 뿐만 아니라 태아 성장과 발달에 영향을 미칠 수 있다.
FDA는 항부정맥제를 5가지로 분류하고 있다.
A: 위험이 없음 (항부정맥제는 없음)
B: 태아에 해로울 가능성이 낮음 (소타롤, 리도카인)
C: 해보다는 이득이 많음 (퀴니딘, 아데노신, 메토프로롤, 프로프라놀롤, 베라파밀, 딜티아젬, 디곡신, 플레케이 나이드, 프로파페논)
D: 태아에 해롭다는 증거가 있음 (페니토인, 아미오다론)
X: 사용금지
우심실 유출로에서 발생하는 특발성 심실빈맥으로 인해 심한 증상 혹은 혈역학적 장애가 동반된다면 약물치료는 베라파밀 혹은 베타차단제가 예방적이다. 약물에 반응하지 않고 증상이 심한 빈맥의 치료로 카테터 절제술이 필요하다. 삽입형 제세동기 시술을 시행 받은 환자는 태아에 문제없이 임신을 유지할 수 있다. 임신 중 제세동기 삽입술이 필요 하다면 방사선 노출을 피하기 위해 피하 삽입형 제세동기나 착용형 제세동기를 고려해볼 수 있다.

4.4 폐쇄성 수면 무호흡증

30대에서 60대 사이에서 조사한 폐쇄성 수면 무호흡증의 유병율은 남자에서는 24%, 여자에서는 9% 정도이다.1 부정맥 발생 유무는 동반된 질환과 주로 연관이 있다. 그러나 돌연심장사와 폐쇄성 수면무호흡증과의 정확한 연관성에 대해서는 아직 논란이 있다. 수면무호흡 증후군은 서맥성 부정맥과 감별되어져야 한다.2 수면 중 호흡장애가 있는 환자에서 수면무호흡이 있으면서 산소포화도 감소하는 것은 돌연심장사의 위험인자로 고려될 수 있다.3

4.5 약물 관련 부정맥

4.5.1 디곡신

디곡신 독성에 의한 지속성 심실부정맥의 치료를 위해 디곡신 항체의 투여를 고려하여야 한다.1,2

4.5.2 약물로 유도된 QT 간격의 연장과 Torsades de pointes

마그네슘 정주로 조절되지 않는 후천성 QT 간격 연장 및 서맥과 연관된 Torsades de pointes 는 심방 혹은 심실의 고속 조율 또는 아이소프로테레놀 투여를 권고한다.3 약물, 저칼륨혈증, 저마그네슘혈증 혹은 다른 인자로 유발된 QT 간격 연장을 동반한 반복되는 Torsades de pointes 환자에게 마그네슘 정주를 권고한다.4,5 후천성 QT 간격 연장과 연관된 Torsades de pointes 환자에게는 칼륨 4.0 mmol/L 이상, 마그네슘을 2.0 mmol/L 이상을 유지하는 것이 도움이 된다.6,7

4.5.3 소디움 통로 차단제 연관 독성

소디움 통로 차단제를 복용하는 환자에서 제세동 혹은 조율 역치의 상승을 보이는 경우 약물을 중단하거나 장치의 출력을 조절하여 장치의 기능을 회복시킬 수 있다.8,9
선천성 혹은 후천성 긴 QT 증후군 환자에서 QT 간격을 연장시키는 약물의 투여는 위험성이 증가될 수 있다.10

4.6 심장이식 후 돌연 심장사

심장이식 후에 좌심실 기능부전을 동반한 심각한 동종이식 혈관병증이 발생한 경우, 환자의 기대 여명이 1년이상이라면 삽입형 제세동기의 사용이 적절할 수 있다.13

4.7 육상선수에서 돌연심장사

육상 선수들은 비육상 선수들에 비해 돌연심장사의 위험에 훨씬 더 노출되어 있다.1 35세 미만의 젊은 육상선수들에서 연간 돌연심장사의 발생은 10만명당 0.7에서 3명에 이른다.2 나이가 들수록 발생율은 더 증가할 것으로 기대되고 활동의 강도와 연령이 핵심적인 위험인자이다. 운동 참여전 스크리닝 검사는 효과적이지만, 그 방법은 매우 다양하다. 연령에 특화된 심장질환 위험인자를 찾도록 심장 스크리닝 검사가 시행되어야 한다. 35세 이하 선수에서의 검사는 유전성 심근병증과 이온통로병증에 초점을 맞추어야 하며, 그보다 연령이 높은 선수의 경우 관상동맥질환이 돌연 심장사의 가장 흔한 원인이므로 이에 대해 검사가 이루어 져야 한다.3

돌연 심장사는 심방세동이 빠른 심실반응을 보이면서 심실세동으로 변형되면서 발생한다. 따라서 부전도로의 순행성 불응기가 짧을 때 위험도가 증가한다.1 빠른 심실반응을 갖는 심방세동으로 인해 심실세동을 초래한 WPW 증후군 환자가 돌연심장사로부터 소생, 회복되었을 때 전기생리학 검사와 카테터 절제술을 권고한다.2 환자가 증상이 있거나 부전도로의 불응기가 240 ms 이하일 때 카테터 절제술을 권고한다.3 전기생리학 검사에서는 심방세동 상태에서 가장 짧은 조기흥분상태의 RR 간격을 측정해야 하고 부전도로의 개수와 위치, 순행성 또는 역행성 특징과 불응기를 확인해야 한다. WPW 환자에서 방실결절의 불응기를 증가시켜 부전도를 통한 순행성 급속 전도를 유도시킬 수 있는 칼슘통로차단제, 디곡신의 투여는 금기이다.

4.9 고령환자에서 돌연심장사의 예방

중증 질환을 가진 고령의 환자에서, 삽입형 제세동기의 일차 예방의 적응증에 해당하고 환자의 기대 여명이 1년 이상이라면 삽입형 제세동기 시술이 용인될 수 있다.1
